Ayurvedic Health Transformation Program

Agreement- 

Use of the services of Nicole Tierre, LLC will be governed by the following Terms and Conditions, please read the following information  FULLY AND CAREFULLY.

Session Length:

The length of sessions will be no longer than indicated below:

Initial Consultation: 120 minutes

Report of Findings: 60 minutes

Follow-Up Session:  60 minutes

Payment/Refund Policy:

Payment is due before the beginning of the first session of the package.   

Payment will be refunded for any reason, upon written request, for any package if the first session has not yet been conducted.

The practitioner reserves the right to choose to end the coaching/consultation process prior to the completion of a package, at her discretion.  In this unlikely circumstance, being unable to come to an agreement after in-person discussion, the practitioner will notify the client of the termination of service in writing, and refund the funds for unused sessions through Stripe within 24 weekday hours.

All packages expire after 6 months of the initial payment, regardless of how many sessions have been used.  Unused sessions will not be refunded.

Cancellation Policy:

Cancellations and rescheduling must be done at least 24 hours prior to the scheduled session time, during normal office hours (Monday - Friday, 10-5 Pacific).  Rescheduling for a valid emergency will be allowed.  Otherwise, sessions cancelled within less than 24 hours prior to your scheduled time will  result in the session not being made up or refunded.

After 10 minutes of failure to log into Zoom for the scheduled appointment, your session will be deemed “Missed.”  Missed sessions are not made up or refunded.  

If you experience an issue with technology or an inability to log into your session for some reason, please inform Nicole Thompson as soon as possible so that she can attempt to help you navigate the system.  She can be reached at (623) 252-3352 in these circumstances.

Commitment:

All forms and information requested by Nicole Thompson must be completed and returned no less than 48 hours prior to the agreed upon session time.  The practitioner needs this time to prepare and review the information beforehand in order to make best use of the allotted session time later.  If forms are not received in time, the practitioner can cancel the scheduled session to give you time to complete the needed information.  Simply rebook when ready.

Chat/Email support is included for every package through the completion of the final session of the package.  All packages (including chat/email support and the ability to book sessions) expire after the final session in the package or after 6 months of initial payment, whichever comes first.

I have read and agree to the above terms and conditions regarding policies and procedures for Nicole Tierre, LLC.  By purchasing this package, I confirm that I have read and fully understand the above and am in complete agreement thereto, and do freely and without duress consent to all terms contained herein.
